If your injury is work related, your employer, case manager or work comp insurance adjuster must make your appointment. We cannot schedule work comp appointments directly with the injured employee.

Authorization & Consent Forms We may release your medical records to share your health information with your written consent. Print and complete the form(s) and submit to our office. Please allow us 7 to 10 days to process your medical records requests.
